"The National Institute for Occupational Safety and Health identifies five common factors in firefighter traumatic deaths. These factors revolve around breakdowns in communications, incident command, accountability, SOPs, and risk assessment.
There is a common thread that ties the five common factors together, firefighters making decisions based on their orientation to the rapidly evolving conditions. When the firefighter is properly oriented (i.e., in tune with the reality of the conditions) and taking intentional action, all goes well often. When the firefighter is out of sync with the conditions, success becomes more accidental than intentional, increasing the likelihood of calamity.
The key is teaching firefighters how to remain properly oriented to their environment through a deep dive into the OODA Loop process and augmenting the process with user-friendly technology that supports intentional action, not accidental success"
